The Credit Card Consent form is used to pre-authorize credit card charges for an appointment. After booking an appointment, you can email a credit card consent form to the cardholder where they can view the appointment details and the terms and conditions of credit card pre-authorization. The completed consent form can be viewed from Scheduler.
Note: Consent forms can only be sent for appointments guaranteed with a credit card.

- From the main menu, navigate to Appointments > Scheduler.
- Right-click on the appointment block.
- Select Send consent form from the context menu.
The Consent Form window opens.
Note: The Send consent form option does not appear if the appointment is not guaranteed with a credit card.
- Enter the card holder's email address and click Send.
The Credit Card Consent form is sent.
